Short-Form Report
A short-form report is a concise document and is designed to quickly convey specific, limited information. A therapeutic short-form report will include the reason for referral, presenting concerns, clinical assessment, treatment, and ongoing recommendations.
Short-form Report fee: $250 per hour. These reports are usually 1 to 2 hours of work.
Long-Form Report
A long-form report is a comprehensive, detailed document that analyses complex issues and features a structured format. This report will include an in-depth assessment and clear recommendations. It is not uncommon for a long-form report to be anywhere between 30 and 60 pages in length.
Long-form report fee:
The fee for a long-form report varies widely based on the purpose of the report and the complexity of the matter.
The cost ranges from $2,500 to over $6,000.
An accurate quote will be provided prior to the commencement of the report.
